Kneecap should be in the middle of the front part of the knee. To be there, two attached structures to the kneecap” IT band in outer side and a small muscle called VMO in inner side” equally pull the kneecap and keep it align.

Imbalances between these two structures, either weakness of the muscle or tightness of the band pull the kneecap to the outer side, which in turn cause pain and cracking sound.
